The sale of CHO Application form for suitably qualified candidates for admission into the School of Community Health, Usmanu Danfodiyo University Teaching Hospital, Sokoto for 2023/2024 Academic session hereby commences on 1st  February 2023 and ends on 28th July. 2023.

The price of the form is Five Thousand (N5,000) naira only for CHO 2 year and (10,000) naira only for CHO I year.

ELIGIBLE CANDIDATES

The following category of candidates are eligible for admission.

•Holders of Diploma in Community Health or Community Health Extension Workers (CHO 2 year) with (Bsc) in Community Health or Health related course will be admitted into CHO I year

•Holders Diploma in Community Health or Community Health Extension Worker (CHEW) Candidates with above certificates MUST possess five (5) credit level passes in English Language, Mathematics, Biology/Health science, Chemistry and Physics in NECO, WAEC OR NABTEB not more than two sittings. Also candidates MUST possess valid license to practice with Community Health Fractitioners Registration Board of Nigeria

The form is obtainable after generating Remita (RRR) and payment of is Five Thousand (N5,000) naira only for CHO 2 year and (10,000) naira only for CHO 1 year to any Bank Into SOCH ACCOUNT, receipt is obtainable from the School Accountant at cash office in the administrative block and collection of PIN and serial NUMBER from the school for online completion of admission form.

Entrance Examination is scheduled to hold on AUGUST 2023
