Please anyone with similar problem with submitting screening form?

when I click update information and filling the screen form, below error display.

The following Subject(s) could not be verified for Medicine and Surgery

and FUOYE UTME for medicine is Eng, Phy, Chem and Biology which I did.

Please what can I do.?
