I had been using my school app, later lost my phone but now have got...

I had been using my school app, later lost my phone but now have got another phone trying to login with my activation code and it's not going through, saying your activation code has been used. but I suggest because didn't log out b4 lost the phone, what can I do pls and pls oo?
