go to the hospital you were born beg them
tell them you need your immunization card that you can't find your old one
but you need it on the old card too
because it's a different one that is been used now

you go pay big cash sha
but it's worth it as far you get what you want sha

if they can't provide you with it
your best chance is to beg them help you fill the new type of card
you go still pay sha lol

As of the time of this post,u could still edit your data.@Tonia,maybe u should upload the Jamb 03 Form you are saying becuz it's only Jamb REG No that u have at the upper right hand of the form.So,where is the three spaces located.
