• All successful candidates MUST present to Nana Babajo School of Midwifery, Malumfashi, the ORIGINAL and 3 copies of the under-listed:

• 1. Admission Letter (To be printed online, COLOURED and on a cream CONQUEROR Paper)
  2. Course Registration Form
  3. Payment Slip
  4. Completed Acceptance Form
  5. Completed ID Card Form
  6. Completed Undertaking Letter
  7. Completed Guarantor’s Form
  8. Six (6) Passport Photographs
  9. Acknowledgement Card
  10. SSCE Certificate or Statement of Result
  11. Secondary School Testimonial
  12. Primary Certificate
  13. Primary School Testimonial
  14. Indigene Letter
  15. Letter of Identification of Residence (If Community Midwifery)
  16. Hostel Fee Payment Slip (for those that will stay in hostel)
  17. Hostel Allocation Form (for those that will stay in hostel)
  18. Result Scratch Card(s)
  19. One (1) A4 RIM Paper

NOTE: Item 2, 3, 4, 5, 6, 15 and 16 can be printed on the Registration Portal.

• Failure to register by Friday 19th March, 2021 means forfeiture of admission.
• Matriculation Ceremony: Monday, 22nd March, 2021.
• Commencement of Lecture: Tuesday, 23rd March, 2021.
