If James fails his examination, his teachers, his parents, his friends, "OR" I ______ to be blame.

If you had "and" before I, the answer would've been are.
But it was "or" before I, therefore the correct answer is am.

The correct answer is "are" not "am"
That's because are is used for a group of people while am is used for only a single person.

I think the answer is B because one person out of the four people the teacher, parents, friend ,or him is to be blame

Compound Subject with "or": When two or more subjects are connected by "or" (or "nor"), the verb agrees with the subject that is closest to the verb.

In this sentence: The compound subject is "his teachers, his parents, his friends, or I". The subject closest to the blank (the verb) is "I".

Verb Agreement with "I": The correct form of the verb "to be" that agrees with the pronoun "I" in the present tense is "am".

it's you "are"
I "am"
To be verbs
