The answer to this is wrong. 'I" is regarded as plural and therefore goes with plural helping verb.

AM is plural verb and so is the correct answer

The selected answer is wrong:

You can't say "I is to blame for the lapses" you can only say "I am to blame for the lapses". That implies that the answer is B, It is I who am to blame for the lapses.

" It is I who AM to blame for the lapses" is correct. Normally the verb in a relative clause takes the third person. However, the verb can agree with the noun referred to in the main clause when the noun is in the first or second person. In this instance “who” refers to “I”, first person, and therefore “am” is appropriate.
