The answer is correct, Have / has(A).
When words like who, which, that are used in a sentence to determine the verb, We check the word before Who, which, that.
For example One of the ladies who ...... pregnant..... Given birth. The answer will be, Were and Has. why ?? Because who points to the noun before it which is what ?? Yes Ladies, and Ladies is What?? Yes A plural Noun, And the law of concord states that Plural noun(Ladies) attracts Plural verb(Were).
while for the other missing space, Has is the answer cause One of them (singular) has(Singular) Given birth.
So for the question,
"Which" points at questions which is plural which attracts Have (plural verb) and "your" which is referring to one person which attract has( singular verb)

This is because a singular noun goes with the pronoun 'has' e.g He has - - -. The quality (singular) has - - -. Now, for the adjectival phrase 'which has always attracted attention', it is still referring to a singular noun.

You guys should go and study your Text please, Myschool is very correct,I am Tired of typing
Study Concord on relative pronoun.

The most appropriate option is:
**C. has/has**
The correct sentence is: "The quality of your questions, which **has** always attracted attention, **has** never been in doubt."
"Quality" is a singular noun, so both verbs need to be in the singular form "has."
